Based on observation and interview, the licensee did not comply with the section cited above in One (1) out of One (1) sleeping infant was not under direct visual observation of a staff person which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observations, records review, and interviews, the Facility did not comply with the section cited above as there were rat feces and dead roaches found in the facility. This poses a potential health, safety or personal rights risk to persons in care. Sanitation regulations with the Administrator. Administrator states facility will send the San Diego Regional Office a written plan of correction on the new cleaning protocol plan and proof of continued pest control services by 12/9/22.

Based on interviews and records reviewed, the licensee did not ensure children in care were free from intimidation, as S1 yelled at children on multiple occasions, which poses and immediate Health, Safety, and/or Personal Rights risk to children in care.

Based on records reviewed, two (2) children’s files were missing immunization records, which poses a potential Health and Safety risk to children in care. This is an amended version of the orginal created on 5/6/22
