LPA observed teacher's aid Frances Jacquez does not have fingerprint clearance. 1 Director Rene Rhodes understands Frances Jacquez be present at the facility until fingerprint clearance is obtained. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

LPA did not observe a current roster of children. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Based on observation, the licensee did not comply with the section cited above. LPAs observed one child walk out of the lunch room, down the hall to the bathroom without supervision. which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPAs staff 3 who is an aide in the two's nap room with eight children. Three children were awake which poses an immediate health, safety or personal rights risk to persons in care.

Based on interview, the licensee did not comply with the section cited above. LPAs learned during visit that four staff tested positive in 2022 and department was not notified which poses an immediate health, safety or personal rights risk to persons in care.
