Based on record review, the licensee did not comply with the section cited above. Lorena Rodriguez Cortez is not associated to facility and was employed 10/05/2022 which poses an immediate health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above. Angela states facility does not provide morning or afternoon snacks to children which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above. LPA's observed during the visit children playing near the sliding door in the playground to the right of the building. LPA Deanna Villagrana opened the sliding door and did not see a teacher present. LPA went around the corner and observed the teacher sitting in a chair to right of the building with no view towards the sliding door. Children were not being supervised which poses an immediate health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above. LPA's observed the facility changed their Parent Handbook and Parent Agreement and did not notify the Department which poses/posed a potential health, safety or personal rights risk to persons in care.

Angela obtained an influenza waiver for staff who were present. Teacher 2 in missing influenza vaccine which poses/posed a potential health, safety or personal rights risk to persons in care.
