Based on [(observation) and (interview) the licensee did not comply with the section cited above in that during LPA's tour with Director, no carbon monoxide detector was observed at facility which poses an immediate health, safety or personal rights risk to persons in care.

Based on [(observation) , the licensee did not comply with the section cited above in that during tour of facility LPA observed debris in classroom and bathrooms sinks areas, toilets and floors which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsiblity for Providing Care and Supervision: a) The licensee shall provide care and supervision ...1)No child(ren) shall be left without the supervision of a teacher at any time...(1). Supervision shall include visual observation. Civil penalty of $500 assesed. Staff was aware children were missing, however, did not follow protocols per staff interviews. Based on LPA interviews, staff did not provide care and supervision for two children who where left unattened at the facility playground. This is an immediate health and safety risk to the children in care. 8 Site Director, will conduct a safety training and name to face protocol during transition from class to playground.

Based on observation, record review, the licensee did not comply with the section cited above in that not all water dispensers at the facility received lead testing for lead leavels and still accessible to the child which posed a potential health, safety or personal rights risk to persons in care.
