(1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met evidence by: 1 Area Director stated they will provide a written preventative maintenance plan and removed the rat remains on the ceiling in the middle classroom by sending a picture to the licensing department by 06/19/2024. Based on observations, review of pest control service agreement and interviews with staff members, and the children stating they have observed rats at the facility. Which poses a potential Health, Safety or Personal Rights risk to persons in care. 8
