This requirement was not met evidenced by: 1 Director states that they will submit a detailed and modified plan to ensure Teacher-Child Ratio are being met at all times. Verification will be sent to LPA Castro via email (alma.castro@dss.ca.gov) by the due date. Based on observations and interviews during 06/09/2026 visit and record review, S1 (who was not qualified as a teacher) was providing care and supervision for ten children in classroom #4 by themselves, which posed an immediate risk to the health, safety and personal rights of the children in care. 8

Director stated they would obtained the signed LIC 9224. At the end of the visit, Director provided copies of five signed LIC 9224. This requirement is not met evidenced by: The Director stated the facility has not obtain the signed LIC 9224 Acknowledgement of Receipt of Licensing Report from children's representatives, which poses potential risk to the health, safety and personal rights of the children in care. 8

Based off LPA's and LPM's observations during 06/03/2026 visit and record review, LPA observed Staff #1 (S1) and Staff #2 (S2) caring for 19 preschool children despite not having the qualifications. Based on record reviews, transcripts reflect that S1 and S2 were not qualified to watch children withiout a qualified staff member. 8
(c) To be a fully qualified teacher, a teacher shall have one of the following: (1) Twelve postsecondary semester or equivalent quarter units in early childhood education or child development completed, with passing grades, at an accredited or approved college or university; and ... Director states she will to submit a detailed plan and schedule on how the facility can ensure that there is a qualified teacher supervising children in care. All teacher's aides must be under supervision of a fully qualfied teacher. This requirement was not met by: During the physical inspection, interview, and record review, LPA observed that S1 and S2 have not completed 12 units in Early Childhood Education or Child Development, which poses a potential health, safety or personal rights risk to persons in care. 8
Based on observation, interview and record review, the licensee did not comply with the section cited above in LPA observed playground slide to be broken and in disrepair which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA Rivas observation, the licensee did not comply with the section cited above by commingling two age groups (total of 10 preschool age and school age children) between 7:30 am - 8:00 am which poses an immediate safety risk to persons in care.
