Based on interviews, facility did not prevent Child #1 from being bitten multiple times, which poses a potential health and safety risk or personal rights risk to children in care.
(b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level.(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedence The facility has scheduled repair of the drinking faucet for December 1, 2023. Work would be completed in January 4, 2024. Director provided LPA a copy of the scheduled work to be done at the facility. Per Director, faucet will be retested once work has been completed. This requirement was not met as evidence by record review. Lead test results showed that one water source had an action level exceedence of 6.8. This is a potential risk to the health and safety of the children in care. LPA Navarro observed the drinking fountain outside was made inaccessible with a plastic bag and has been shut off. 8
