Based on observation and record review, the licensee did not comply with the section cited above. During tour at 2:00pm, LPAs observed 2 staff that were not on the Department roster. At 2:20pm, Director provided proof of clearance. At 2:21pm, LPA Sanchez call the Regional Office for more information. Staff #2 and Staff #3 are not cleared. This poses/ posed a potential health, safety or personal rights risk to persons in care.
record review. Lead test results showed that one water source had an action level exceedence. LPA Navarro observed the sink faucet had signs next to it stating that the faucet is out of order. Per Director, the drinking fountain is not used for food 8 preparation or for drinking.
