Director informed LPA that they closed their infant license and ceased operations on 03/21/25, and LPA received letter of closure/non operation from licensee today. Deficiency cleared at time of LPA visit. Director did not comply with the section cited above as witness and interview statements revealed that on at least one occasion, a child from the infant license was taken to a preschool classroom due to staffing issue, which poses a potential Health, Safety, or Personal Rights risk to persons in care.

Director stated they will schedule another teacher to come in at 8:15AM to ensure proper teacher to child ratio. LPA will conduct a Plan Of Correction visit. During today's visit at around 8:27AM, LPA observed 25 children in care with 2 staff, and no other staff available to step in to help ensure proper preschool ratio of 1 to 12. LPA observed outdoor play with 1 staff and 13 children in care until Director came in to help around 8:50AM.

LPA observed a child go in from the playground without staff knowledge. LPA walked the child over to another staff member inside the facility, and child stayed with staff indoors until another staff came in to help, which poses an immediate health, safety, or personal rights risk to persons in care.
