Based on interviews and a self-report from the facility, staff did not maintain visual supervision of a child for approximately 1 minute which poses/posed an immediate health, safety or personal rights risk to persons in care. 8 Facility will provide a plan of action on how they will execute maintaining constant supervision. An immediate civil penalty of $250.00 has been assessed on today's visit.

Based on records review, interviews, and self-report from the facility, the facility did not maintain visual supervision of Child 1 (C1), for approximately 3 minutes, while C1 was on facility grounds. This poses/posed an immediate health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in that none of staff had a current mandated reporter certificate on file which posed a potential health, safety or personal rights risk to persons in care.
