Based on observation and interview, the licensee did not comply with the section cited above in the border around the carpet in room K1 is cracked, the ABC rug is lifting/curling, the white fence in the entrance of the facility has rust and deteriorating signs since its missing some metal parts and doesn't close properly, and the cement ramp with railings made of wood needs replacement due to not being sturdy and is falling apart which poses a potential health, safety or personal rights risk to persons in care.
