During the inspection, the facility representative worked with human resources department to transfer the fingerprints for S1 thru 4 and confirmation was provided to LPA. According to facility representative, moving forward all transfer requests will be emailed to her and HR director for processing to ensure that requests are processed in a timely manner. LPA observed that fingerprints for staff 1 (S1) thru 4 were not transferred/associated to the facility.

Staff 2 and Staff 3 witnessed 1 Per Assistant Director, All staff will be additional training regarding personal rights of children. Once training is complete, proof of training will be forwarded to the department. Staff 4 pulled the arm of C1 roughly. C1 also revealed that Staff 4 pulled their arm.

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirment was not met as evidence by: 1 Per director the drinking fountain with lead exceedance was removed on 03/15/2022 and water source was capped. LPA did not observe drinking fountain in the playyard. Based on record review, the facility had a high lead exeedance of 240.000 UG/L that poses a potiental health and safety risk to children in care. 8
