Per facility representative, all staff mandated reporter training was conducted on 08/14/26 and covered mandated reporter Q & A, scenarios, reminders, and provided the mandated reporter website. According to the facility representative, staff take the mandated reporter training every year. Staff did not report suspicion of neglect or abuse regarding C4 which posed a potential health, safety or personal rights risk to persons in care. 8 A copy of the training agenda with staff signatures was provided to LPA.
Per facility representative, S1 was transferred/associated to the facility and a clearance letter was provided to LPA. HR will check temporary employee files to verify that fingerprints have been processed moving forward. LPA observed that staff 1 (S1) fingerprints were not transferred/associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.
Per facility representative, staff should be moving them as soon as the infant is as sleep. The facility will do a retraining of safe sleep and practices for the infant staff or anyone working in the infant program. A sign in sheet will be sent to LPA by
During the inspection, the facility representative worked with human resources department to transfer the fingerprints for S1 and confirmation was provided to LPA. According to facility representative, moving forward all transfer requests will be emailed to her and HR director for processing to ensure that requests are processed in a timely manner. LPA observed that staff 1 (S1) fingerprints were not transferred/associated to the facility.

Staff #2(S2) gave C1 low fat milk during breakfast time not know C1 is lactose intolerant. Medical records and Parental Request iindicated C1 is to be provided lactose free milk.
Per Director, All staff providing care and supervision to children will be provided additional training regarding personal rights of children. Once training is complete, the Director will forward proof of training to the department. Five of eight staff interviewed indicate that on more than one occasion they have witnessed S7 mistreat infants. Two of eight staff interviewed also indicate that on more than one occasion they have witnessed S8 mistreat infants. S7 admitted to mistreatment of children. 8 POC already received. Cleared during site visit.
