(a) The licensee shall provide care and supervision...(1)No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met evidenced by: 1 Per interim director, disicplinary actions were taken for 3 teachers, staff involved recieved re-training on supervision and transitions. Per Interim director, proof of training with dates and staff signatures will be emailed to LPA S Li by 7/10/26. Based on record review and statements obtained, one child was left in the play yard after classroom transitioned inside for 1-2 minutes, which poses a healthy and safety risk to children in care. 8
Based on interview, Licensee did not ensure children's relationships with staff were accorded dignity, as Staff #4 was disclosed as roughly handling children and displaying inappropriate behavior in front of children. This posed an immediate risk to the health, safety, & personal rights of children in care 8 signed attendance sheet, will be submitted to LPA by POC date. POC date is 07/10/2024, due to Center Director being unavailable until then.
