Based on observation, an infant child (C1) was left sleeping in a crib in the infant room with the door closed and no staff member present in the room, three staff members were preoccupied feeding infants in the hallway in the other room which poses an immediate risk to the health, safety, and personal rights to persons in care.

Based on observation, the licensee did not comply with the section cited above in which C7 was left unattended in a high chair for approximately 5 minutes around a corner while S1 was loading other children into a stroller which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee has made the faucet temporarily inacessible by shutting the water off, and children receive drinking water from bottled water. Licensee is fixing the faucet on 09/22/23 and will re-test. Retesting documents will be submitted within 2 of the completed sampling. On 09/07/23 licensee failed to maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for site "D". Water testing results indentified with Action Level Exceedence as defined in WD section 101700.3 are not deemed safe to drink. This is a potential health and safety risk to children in care.
