Facility Representative provided a copy of the updated written policy for transitions, along with training agenda for the training conducted on 07/14/2026 . Deficiency was cleared during inspection. C1 was left on the playground without staff present during a transition to the classroom. This poses an immediate risk to the health, safety and personal rights of children in care.
Director has submitted a Plan of Correction regarding the use of children's personal belongings and clothing to ensure that children's personal rights are not violated. This deficiency is now cleared. On 5/29/26, Staff member dressed a child in garment that belongs to another child without prior approval from parents/ responsibility parties, which poses potential health, safety or personal rights risk to persons in care.
