Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the review of staff files the LPA was unable to review current immunizations for S2 and S4.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the review of staff files the LPA was unable to review an LIC 503 Health Screening for S1.

Based on the observation/interview/record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arriving to the facility the LPA was greeted by the kitchen staff who stated the Director was not present. When the Director arrived she stated she left in December 2024 and is only substituting this week, however, the Department has not been notified of the Director change.

Based on observation, the licensee did not comply with the section cited above, outdoor play equipment had some cracks and rough edges, which poses a potential health, safety risk to persons in care.

This requirement is not met as evidence by: 1 Director agrees to remove the water fountain on outlet B and the sink will be used by staff and children for hand washing only. Director will submit photos when fountain is removed by POC date Based on records review of required lead testing, the facility had lead values of 5.5 ppb or above on water outlets: B (15) & D (6,1). This is a potential health and safety risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in 2 out of 2 floor panel/ transition strips that are missing in one bathroom and broken in the other bathroom in Room C which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation the licensee did not comply with the section cited above in that the red/yellow play structure has unsafe swings and an unsafe elevated walking area. which poses/posed a potential health, safety or personal rights risk to persons in care.

... 1 Licensee agreed to complete training with the staff on how to account for all children within care combined with the importance of providing supervision per regulations of Title 22. A Civil Penalty will be issued due to absence of supervision. LPA Zeron observed a written statement in staff members file regarding a child being left unattended in the classroom on 06/03/2022. This is an immediate risk to Health, Safety & Personal Rights to the children in care.
