Interviews and documents reviewed and received showing, on 09/23/2024 in the morning, there were only 1 Per Director she is actively trying to hire additional teachers/teacher aides. Per director she will revise the staff to child ratio schedule and will submit a copy to the department, as well as be trained and train the staff regarding teacher child ratio by reviewing the state laws and regulations 2 teachers with 33 children in one class room before the next staff person arrived to work. This poses an immediate risk to the health and safety of children in care 8 and the department website to review the teacher child ratio videos and submit a copy of the staff names and dates of when each staff member was trained. LPA obsered a toilet not working properly and per interviews it was stated one or more toilets are always 1 Director states will have the maintenance service repair the toilets and will submit a copy of the work orders. Director will also submit a documented new procedure for ensuring there is enough daily supplies such as hand soap are always available. POC date 10/11/2024 not working properly or not at all. Interviews also stated that supplies such as soap to wash hands are not always available. This poses an immediate risk to the health and safety of children in care

LPA observed tiles on the ceiling in several of the class rooms and 1 Per conversations with the Director and Administrator during the walk though it was stated that they have already placed work orders to have several items at the day care fixed and some of the items being fixed are the Plumbing, Roof, Electric, wood on bungalows for termites & parking lot repaving hallways to the class rooms had stanied water damaged and some tiles hanging down from the roof from rain buildup. LPA also observed a couple of toilets not working properly (Flusing slowly). This poses a potential risk to the health and safety of children in care. 8 LPA was provided with several documents for all the items listed that are being fix. All items will be completed by the end of March 2023 and facility will provide proof of all items completed.
