Based on the interviews with staff, parents, and children,and the physical plant inspections, 1 The LPA discussed a correction for this violation with owner/director. They stated they will email the LPA a detailed plan for how supervision of children will be maintained by staff at the facility at all times. This plan will be emailed to LPA by 10/5/21. Title 22 regulation section related to this violation was provided and reviewed. LPA has determined that children were not supervised correctly on the date 8/3/21 at approximately 1pm on the outdoor play yard, which led to an innappropriate interaction between 3 children. This is an immediate threat to the children's health and safety.

Based on LPAs observation of the classroom and census taken, it was determined that one teacher was supervising 15 school age children. This was observed to occur during the inspection by LPA on the morning of 10/5. 1 The LPA discussed a correction for this violation with owner/director. They stated they will email the LPA a detailed plan for how supervision of children and teacher to child ratio will be maintained by staff at the facility at all times. This plan will be emailed to LPA by 10/5/21. Title 22 regulation section related to this violation was provided and reviewed. This is an immediate threat to the health and safety of the children in care.

It was further determined that a staff #4 was performing house keeping duties at that time. This is a potential threat to the children's health and safety.
