A child safety gate will be installed to Room A backdoor. Based on staff interviews and child#1parent interview, child#1 was found outside the facility by the sidewalk of the driveway of the school on 10/24/23 approximately at 12:05-12:07 PM. Staff did not see how child#1 left the school. This poses an immediate health, safety and personal right risk to the children in care. 8 $250 Civil Penalty was assessed on this due to repeated violation within 12 months.
