Based on observation, interview, and record review, the licensee did not comply with the section cited above. While reviewing infant safe sleep documentation, LPA observed safe sleep logs for 2 out of 8 children. Staff stated they are only documenting infant safe sleep for infants under 12 months. During children file reviews at 11:27 AM, LPA observed an infant under 12 months of age did not have sleep logs available either which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above. LPA observed infant changing table to not be within arms reach of the sink which poses/posed a potential health, safety or personal rights risk to persons in care.

While touring the infant room at 10:08 AM, LPA observed children bottles not labeled with the current date and one bottle was not labeled with the infants name and the current date which poses/posed a potential health, safety or personal rights risk to persons in care.
