Based on interviews from the four staff, review a video footage, one staff statement, it was disclosed that staff did not observe how the injury occur, which poses a potential risk to the health and safety of children in care.
Based on LPA interview with the staff members on 06/18/2024, a child suffered a broken elbow due to a fall. Facility documented the incident but did not report it to the local licensing agency, which is a potential risk to the health, safety, or personal rights of the children in care.
(e) As a condition of licensure, the areas around and under high climbing equipment...that absorbs falls. (1) Sand, woodchips and pea gravel, or rubber mats... This requirement was not met: During today's inspection, LPA observed new woodchips under and around the climbing equipment, slides and other similar equipment. Based on observation the cushioning material of woodchips under and around the climbing equipment, slides and other similar equipment appeared to not be sufficient to absorbs falls. This poses a potential risk to the health, safety or personal rights risk to the persons in care. 8
101700.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 Director made outlet in Room B116 classroom inoperable by means of a sign that reads "Do Not Use." Director stated the outlet will be turned off until replaced and re-sampled. Director stated she will provide proof of replacement and re-sampled documentation via email by 10/7/22. Based on lab report analysis water from outlet L (7.70 parts per billion) This outlet samples taken on 08/10/2022 indicate levels of lead in exceedance. 8 Filtered water pitchers from the other PASS sinks, along with individual bottle waters readily available for the staff and children.
