Based on observation, the licensee did not comply with the section cited above in that LPA observed three wood panels located under the patio that had sharp nails, that area accessible to children in care. LPA also observed that there is a potential tripping hazard by the water hose located in the backyard, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that LPA observed that two staff members Staff 4 and Staff 5 did not have their LIC503 completed, which poses/posed a potential health, safety or personal rights risk to persons in care.

information obtained from the 1 Licensee to have an all-staff training regarding care and supervision of children and provide a written copy of the curriculum and sign in sheets showing who attended. Plan and scheduled date of training is to be submitted to Community Care Licensing by 5/22/23. facility. On 5/16/23 facility self-reported that on 5/12/23 Child #1 was left unsupervised outside for no less than 30 minutes. This poses an immediate risk to the health, safety or personal rights of children in care. A civil penalty is being issued.

information obtained during the investigation that children do not have water readily 1 Currently the center uses Brita pitchers that get filled in the kitchen. Through interviews conducted it was revealed that the pitchers take a while to get refilled leaving the children without water to freely drink. Licensee to create a plan and submit to community care licensing ensuring how water will be readily available to freely drink when playing outdoors. This poses a potential risk to the health, safety or personal rights of children in care. 8 available for children to freely drink at all times. HSC
