The Department was notified that Child 1 and was left unsupervised by a total of three (3) staff for approximately 5 minutes, which poses an immediate health, safety or personal rights risk to persons in care. 8 LPA informed Licensee that the Department will be in communications with Licensee for an in-office meeting at the Fresno Regional Office.

Based on record review and interview. The Department was notified that Child 1 and Child 2 were left unsupervised by Staff 1 for approximately 2 minutes. This poses a potential risk to the health, safety, or personal rights of children.
Based on record review That Staff #1 was given a Correction 1 LPA reviewed documentation that revealed that facility held a staff meeting regarding these incidents and corrections. Site Director provided LPA with copies of Correction Action for both Staff #1 and Staff #2. Action Notice for sleeping on the job and Staff #2 was also given a Correction Action Notice for, Neglect of duty. As stated in LIC9099. This poses a potential risk to the health, safety, or personal rights of children.
