Director states that she will conduct an all staff meeting reqarding incidents that are required to be reported per licensing regulations. Director states she will send a copy of the signed agenda to the department by 6/15/26 During an investigation it was discovered that the facility failed to report two incidents that occurred on 4/20/26, where in two children were unsupervised for approximately two mintues. This poses a potential health, safety, and personal rights risk to children in care.

(a)The licensee shall ensure that each child is accorded the following personal rights: (3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse... This requriment was not met as evidencyed by: 1 Director states she has a staff meeting scheduled for 06/13/26 regarding personal rights. Director states she will send a copy of the signed meeting agenda by POC date 6/15/26. Based on staff interviews, it was disclosed that staff (S8) physically restrained a child (C1) and held them down in their cot against there will. LPA determined that this is a potential health, safety, and personal rights risk to children in care. 8
