LPA observed the staff removed the cloth material that was wrapped around the 2 infants. At 10:10AM during a tour of the infant room, LPA observed 2 infants that were swaddled. One infant was held by a staff and another infant was napping. 8 Statement of Plan of Correction must be submitted to the Department no later than end of business day on 10/11/23.

Based on interviews conducted by LPA several classrooms and carpets/rugs within the facility smell on occasion and are in need of cleaning and/or repair. This poses a potential, health, safety or personal rights risk to the children in care.

Based on observation and interviews conducted by LPA several classrooms and carpets/rugs within the facility smell on occasion, and some of the panels on the roof of the outdoor walkway between the classrooms are missing. This poses a potential, health, safety or personal rights risk to the children in care. 8 The director also states that she will create a written schedule/plan for repairs to the outdoor covered walkway. The director will submit the documents to LPA via email by 6/21/23.
