Based on observation, nterview and record review, the licensee did not comply with the section cited above in the facility was unable to provide proof of testin gon a water outlet located in the facility kitchen, where the facility prepares food, distributes food and water for consumption which poses a potential health, safety or personal rights risk to persons in care.

Based on record review for S4 it was documented that he/she was hired as a Teacher. However, after LPA reviewed S4's transcripts and looking up the course decription it was determined S4 has not completed required core classes. S4 is not currently enrolled which poses a potential health, safety or personal rights risk to persons in care.

Based on review of medication LPA observed expired medication for C4 which poses a potential health, safety or personal rights risk to persons in care. C4 was never given expired medication and was not in attendance during inspection.
