Responsibility for Providing Care & Supervision: (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement is not met as evidence by: Based on staff interviews, S2 and S3 stated Child 2 (C2) was left unattended without 1 Director stated the former teacher #1 (FT1) was fired on 10/10/22 shortly after the incident with C3 occured and was not aware of the other incidents. Director has since implemented a new "Head Counrt Policy" to ensure teachers keep track of students and maintain ratios. Director will email supervision in the hallway allowing the c2 to climb up the stairs. No supervision was provided. Director stated she gave Emmy Gorge the paperwork to ger fingerprinted but she never returned it. Since LPA visited on 6/7/23, she sent Emmy home and did not have her return until she was cleared. Civil Penalty of $500.00 was assesed for uncleared staff Emmy Gorge. Staff Member Emmy Gorge was hired in March 28, 2022 and did not have background clearance until June 2023. 8 Director understands that all new staff must have fingerprint clearance prior to their start date.

Director will email LPA Valdez Santana the POC via email at dianna.valdezsantana@dss.ca.gov by due date. DIrector will give children's authorized representatives the LIC 9224 form. During staff interviews, Staff 2 and Staff 3 disclosed that teachers break each other often and combine classrooms, sometimes having up to 14-15 children with one staff supervising. 8 Director stated the facility has reduced numbers and teachers no longer need to break one another or be out of ratio. She has hired more staff since June 2023.
