The children are strapped inside a multi-seated stroller in the lobby, or put in one classroom, or outside on the same play yard which is an immediate health, safety, or personal rights risks to the persons in care.

Based on interviews and record reviews the original, designated director never worked at the facility and the Licensee did not notify Licensing of the changes with the Director which is a potential health, safety, or personal rights risk to the persons in care. 8
