Based on observation and interview the licensee did not comply with the section cited above in Class room #1 and the kitchen had chemicals accessible to the children that were removed but the lock was broken in the classroom and the kitchen needed to have a lockable storage area which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in S5 and S6 did not have a report in their files during inspection which poses a potential health, safety or personal rights risk to persons in care.
Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. Based on the interview and record review this requirement was not met as: 1 The facility director had removed the toy kitchen set from the classroom on the day of: 05/15/2024. Per director statements staff will continuously check on all play equipment to ensure proper safety for the children. LPAs observed the toy kitchen set in the classroom to contain a nail that was sticking out. This poses immediate health, safety, or personal rights risk to children in care. 8
preschool children and school age children commingled in the morning for less than an 1 hour due to short staffing. 1 Directors have hired additional staff members for the school age and preschool license. The facility seperates the preschool children as well as the school age children in their seperate classrooms. This is a type A deficiency which poses an immediate Health, Safety or Personal Rights risk to day care children in care.
the director provided LPA Alemoh with a work order from a Heating and Air Conditioning company indicating the work order being placed on December 8, 22 1 The facility has fixed the heater back on December 20th 2022. LPA toured the facility and checked the thermostat and found them to be working in proper condition. POC cleared during the visit. and fixed on December 20, 22.Temperatures in the classroom ranged from 60 to 61 degrees. Children that were in care during this period did not show any signs of illness. This is a type B deficiency which poses a potential Health, Safety or Personal Rights risk to day care children in care.
