Based on interviews with the director there was some children that did not have files at the facility, including their emergency contact information. 1 Director now has emergency cards for each child located in a central box that staff can access. Deficiency cleared at time of visit. This poses a potential health, safety, or personal rights risk to persons in care.

The director states the facility will hire qualified staff to assist if the director is in a different location of the children or in cases of an emergency. LPA will return to clear plan of correction. Three children were in the back left room with no staff supervision playing as the director was changing a child's pull up in the bathroom which is an immediate health and safety risk to the children in care.
