Based on interview, record review, the licensee did not comply with the section cited above in the facility failed have its drinking water tested for lead contamination levels prior to 01/01/2023 on outlets used for water consumption and food prep in the facility classroom sink, located in B1. In addition, the facility failed to test the 5 water fountains observed in the outdoor play areas on the East and West, playgrounds, used by daycare children, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in the facility sink used for food preparation and handwashing in classroom B1, does not have hot water which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in the 1 of 5 stalls in the designated restroom for daycare children is non-operable and 3 of 5 stalls were observed to have sitting water on the restroom floor, due to broken or faulty pipes, which poses a potential health, safety or personal rights risk to persons in care.
