(D) Any suspected physical or psychological abuse of any child. This requirement is not met by: The director stated they will review the regulations regarding reporting requirements, write a statement acknowledging their understanding of the regulations, and email it to the LPA. Based on observation, interview, and record review, the licensee did not comply with the section cited above. The Director did not report that law enforcement (Garden Grove Police Department) was present at the facility on 09/03/2026, which posed a potential health, safety, or personal rights risk to persons in care. 8

Based on record review of children’s file and facility’s Parent Handbook, the consent form stated parents gave permision for the school to use child photographs and video for the school's promotional purposes and the form was dated for the school year of 2020-2021. This poses a potential risk to the health, safety or personal rights of the children in care. Director submitted the required document on this date.
