(1) No child(ren) shall be left without the supervision of a teacher at any time, 1 Facility will continue to use Numbers when the teachers take the children to the restroom and will write the number on the white board. The teachers will take the number card to the restroom so they will remember the number of the children they took, A video will be sent. This requirement was not met as evidence because (C3) was left in the restroom unattended. 8
101229 Responsibility for providing care and supervison (a) The licensee shall provide care and supervision as necessary to meet the children's needs (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 Licensee agrees to provide the Department with a written plan on how facility will provide training to staff regarding care and active supervision of the children. Licensee will meet with staff and submit signed a signed roster. Based on observations and interviews, Staff 1 (S1) left Child 1 (C1) unattended in the bathroom, which poses an immediate health and safety issue to persons in care. 8
