(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation." 1 Per director, she has had conversations with staff in classrooms, and parents of involved child. She went over how to properly use infant feeding chairs and held disciplinary actions for involved staff. Based on interview, the facility did not comply with the section cited above in leaving a child unsupervised on unbuckled feeding chair causing him to fall, which poses an immediate health, safety or personal rights risk to persons in care.

All classrooms are also required to fill out ratio boards for headcounts inside and outside the classroom for outdoor play time. Director plans to have a training on 8/29/2025 regarding this matter. Director will email LPA copy of powerpoint training. Based on interview, the facility did not comply with the section cited above as three facility staff interviwed stated that there was a child left alone inside the TK classroom during outdoor play, which poses an immediate health, safety or personal rights risk to persons in care. 8
