the incident that occurred on 05/26/23. C1 was left unattended by staff. Child was left outside of the classroom (Room 61) in the playground walkway. Disclosures were made verifying that the incident did occur. Disclosures were consistent. This was an immediate risk to the health and safety of the child in care.

They will submit by POC date. Based on record review, the facility has 1 water outlet that has a lead exceedance of 14.00ppb.
