Based on interviews conducted the facility did not meet the needs of child #1, child #2 on 1 Director stated an all staff meeting will be conducted to discuss how to prevent personal rights violations of children. Licensee will submit to the Department no later than 05/01/23 a copy of the all staff meeting agenda including the meeting sign in and out with the signatures of staff that participated multiple occassions child #1 threw toys and objected to these children. The incident did not result in any injuries. This is a type B deficiency which poses a potential Health, Safety or Personal Rights risk to persons in care. 8 in the meeting.

Based on interview, it was determined that C1 and C2 exited the facility and wandered off. The directors disclosed that this incident was not reported in the required 24hours. This poses a potential risk to children in care.
