Based on interviews conducted it was revealed that the preschoolers and the toddlers were commingled in the morning during the drop off and at the end of the day during the pick up. This is an immediate health and safety risk to children in care. This center is a licensed preschool facility with a toddler option.

Based on record review, the licensee did not comply with the section cited above where one staff was not associated with the facility which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 3 staff were not associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above as LPA observed a classroom with 3 teachers that did not has any ECE courses which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as 6 out of 8 teacher did not has complete immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidence by 1 Director stated that she will request the exception from the licensing office when transitioning children over to the toddler program prior to them turn in to 18 months. Director will also do this for all future transitions. Based on LPAs observation,interview and record review it was determine that a 16 months old child was enroll in toddler component prior being 18 months. This is a potential health and sefety risk to children in care. 8
