Based on interview and record review, it was determined that Child #2 was injured by Child #1. This poses a potential risk to the health, safety, and personal rights of children in care. 8 LPA obtained copies of these plans and Site Supervisor stated facility will continue to reassess support plans as often as necessary, and continue to shadow Child #1. POC cleared today, 05/03/2023.
Based on record review, four outlets at the facility contained lead exceedance greater than 5.5 ppb. This poses a potential health, safety or personal rights risk to persons in care. 8 Facility notified all authorized representatives in English and Spanish of lead testing results. The results have also been posted at the facility where authorized representatives can view them. Facility is providing alternative drinking water in affected classrooms with portable water jugs, indoor and outdoor.
