Based on record review and interview, facility did not ensure Child #1 was supervised by facility when releasing Child #1 to an incorrect individual, who then left facility premises. This poses an immediate risk to the health, safety, and personal rights of children in care. 8 Facility agreed to send the log and outline to CCL by 06/30/2022. Facility was given notice of informal conference scheduled with CCL to discuss concerns. Immediate Civil Penalty was assessed for the day 05/25/2022 in the amount of $500.
