The Site Director states that she will conduct training with the staff members to remind of how the table shall be used and conduct continued observations of the classroom. The staff in classroom 14 did not use the high chair seating equipment appropriately, as infants were observed by LPAs to be seated at the table for elongated periods of time without being fed or being offered other activities, which poses a potential risk to the health, safety, and personal rights of children. 8 The Site Director states that she will consider adding additional ball pits to the classroom for the infants to engage in as well. A written statement will be submitted regarding next steps with staff after review with the Regional Director.

The Site Director shall remove the mobiles from above the cribs and remind staff that no items shall be hanging above cribs. Photos of mobiles removed shall be submitted to the Department by 4/12/2024. The Site Director did not ensure that there were no objects hanging above the cribs as four (4) mobiles were observed to be hanging over the infants cribs in classroom 22, which poses a potential risk to the health, safety, and personal rights of infants in care.

The facility to appoint an infant assistant director and submit qualifications for review. LPA advised reviewing Section 101415.1 for assistant director qualification requirements. The facility has not appointed an assistant director and during visits conducted on 1/9/2024 and 3/12/2024, there were more than 25 infants in attendance, which poses a potential risk to the health, safety, and personal rights of infants in care. 8 Assistant director packet to be submitted to the Department by 4/12/2024 and the person appointed should be aware of their position.

101429 Responsibility for Providing Care and Supervision for Infants (a) ..the following shall apply: (2) Sleeping infant(s) shall be directly observed by sight and sound at all times (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: This requirement was not met as evidened by: 1 The Site Director shall train her staff on proper nap check documentation protocol and remind that infants shall be physically checked every 15 minutes. Nap log shall be completed as checks are done on the infant. The staff in classroom 22 were not physically checking on sleeping infants every 15 minutes as nap check logs were not completed and LPAs did not observe any staff members observing sleeping infants, which poses a potential risk to the health, safety, and personal rights of infants in care. 8 Training review to be submitted and signed by staff members in classroom 22 by 4/12/2024.

The Site Director shall post required temperature signs next to the thermostat in the classrooms, reminding staff to set between 68-85 degrees. Staff shall also have ability to adjust temperature on thermostat. The facility did not maintain proper temperature for children in care as classroom 20 was observed to be 62 degrees when LPA Nelson observed the classroom, which poses a potential risk for the health, safety, and personal rights of children in care. 8 Photos of labels for infant classroom thermostats to be submitted by 4/12/2024.
