101439.1 Infant Care Center Sleeping Equipment (f) Cribs shall be free from all loose articles and objects, including blankets and pillows. 08/07/24 staff removed the small blanket from the child's crib, infant was asleep. POC was cleared immediately staff removed. LPA Breaux noticed during the health and safety tour, that a blanket was in an infant's crib while sleep. Which poses a potential health, safety or personal rights risk to persons in care. 8 Assist. Director will have all staff in that classroom watch and review the Safe Sleep video and information provided to the asst. director on safe sleep. LPAs observed 14 infants in the care of one fully qualified teacher and two aides which poses a potential risk to the health, safety, and personal rights to children in care. 8 at lorraine.dacanaybreaux@dss.ca.gov no later than the due date of 05/31/23.
Based on interviews, it was stated they are not providing 100% visual supervison at all times to children in care which poses an immediate health and safety risk to children in care. THIS IS A ZERO TOLERANCE VIOLATION AND A $500 CIVIL PENALTY IS ASSESSED TODAY AND $100 PER DAY WILL BE ASSESSED UNTIL CORRECTED.
101616.5 Teacher-Child Ratios There shall be a ratio of one teacher to every three infants in attendance. (1) An aide as specified in Section 101216.2 may be substituted for a teacher when all of the following criteria are met: (A) A fully qualified teacher as specified in Sections 101616.2(a) and (c) is directly caring for and supervising no more than three infants; and has overall responsibility for supervising a total of no more than 12 infants, with the assistance of three aides as specified in Section 101616.5(b)(1)(B). 1 During the visit, LPA had the director place the qualified Infants teachers in the classrooms at all time. Director will write a plan to ensure staff is in the correct classrooms and within ratio. Director will complete staff files and contact LPA to review by 05/11/23. This requirement is not met as LPA obsevered and files reviewed that staff was not quaified. Therefore this poses a potential health and safety risk to children in care. 8 Director will email the written plan via email Lorraine.Dacanay-Breaux@dss.ca.gov
101438.1(4)(a) When infants are in care, the diaper-changing area, where residue is splashed, shall be washed and disinfected after each diaper change. (a) The immediate floor and wall areas shall also be kept clean. 1 During today's inspection, LPA had the faciltiy clean, disfect and change the diaper station. LPA inspected the area and the area was satisfactory. Facility representative will create and write a plan of prevention ensure the diaper changing stations are sanitary. Base on observation of the current condition of the diaper changing area,the facility did not ensure this area was clean, which poses an immediate risk to the health and safety to children in care. This is an potential risk to the Health and Safety or personal rights to persons in care. 8 Per owner the entire diaper station will be replaced, the station has been ordered and will take a week to arrive. Facility representive will contact LPA to inspect by 05/08/2023.
