PERSONAL RIGHTS-The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature. 1 All staff are to review the Community Care Licensing (CCL) video CHILDREN'S PERSONAL RIGHTS IN CHILD CARE. Director is to submit a roster signed by all staff that they have reviewed the video. This requirement was not met as evidence of interviews. Staff physically placed her hand on child's leg inappropriately and raised her voice/yelled at day-care. This poses a potential risk to the health, safety or personal rights to children in care. 8 Signed roster is to be submitted to the CCL office by May 19, 2022.

REPORTING REQUIREMENTS UPON the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Dept. by telephone or fax within the Dept. next working day & during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Dept. within 7 days following the occurrence of such event. 1 The Director and all staff will review the CCL video REPORTING REQUIREMENTS FOR CENTERS. Director will document training for staff files and provide CCL with a signed roster of all staff who have reviewed the video. This requirement was not met as evidence by record review: Facility failed to report an unusual incident of a child experiencing an asthma attack and required medical attention. This poses a potential risk to the health, safety or personal rights of children in care. 8
