Board will do practice fire drills to ensure that everyone is familiar with the practice. (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met evidenced by: Staff interviews revealed that Staff #1 and Staff #2 neglected to change children's diapers 1 Director stated that staff #1 and staff #2 are no longer employed here. Director stated personal rights will be reviewed during staff interview. Direct will submit proof to LPA Tillery. in a timely manner, resulting in serious diaper rashes. Interviews also revealed that staff #1 and staff #2 allowed the children to sleep in wet clothing, after outdoor water play. Interview revealed that staff placed a childs wet clothing in the trash instead of with the child's belongings. 8

Supervision shall include visual observation. 1 Director stated that staff #1 and staff #2 are no longer employed here. Director stated that supervision will be reviewed during staff interview. This requirement was not met, evidenced by: Staff interviews reveal that Child #1 left the outdoor playarea and was able to exit the facility out of the main entrance before being stopped by Staff #3. Staff interviews revealed on a separate ocassion, Child #2 was forgotten on the outdoor area for approx. 3 min. before teacher returned for Child #2. 8
