Based on observations and interviews 1 Staff 3 no longer works at the facility. Director will conduct a training with staff regarding discipline policy and staff communication. Director also stated she will contact Child Action regarding staff training as well. Based on observation and interview, a child was hit with a ruler and children were forcibly put on time out, which poses an immediate health, safety or personal rights risk to persons in care. 8 Director will send LPA signed meeting notes and will forward email to Child Action.

Based on observations and interviews 1 Director will speak with staff about personal rights and have Child Action come out and do a training with the teacher on behavioral training and classroom management. Director will send statements from staff that they understand personal rights. staff members can be heard yelling at children. Interviews stated that staff members hold them "really hard" to take them to time out and that one staff member pulls their legs hard, which poses an immediate health, safety, or personal rights risk to pesons in care.
