Based on LPAs observation during drop off and nap time, and interviews, one teacher was supervising more than 12 children. The license did not comply with the section cited above in 3 of 3 open classrooms which poses an immediate health, safety or personal rights risk to persons in care.
LPA Carus observed floors in 2 areas children can access in the facility that are weak, and floors in one of the children’s classrooms that are broken. LPA Carus observed an excessive amount of termites in the children's classrooms. 8 Licensee will submit documentation of the receipt of the scheduled termite treatment to the department by 12/13/2025, and agrees to have ongoing treatments if necessary in order to eliminate the termites permanently.
