LPA and Site Supervisor observed the changing table in the infant room was not within arm reach of the sink ,while staff was changing a child, which poses a potential health, safety or personal rights risk to persons in care.

LPA and Director observed the cleaning/disinfectant containers and dispensers installed on the wall, right necxt to the hand washing sink in the Toddler bathroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
