Center Supervisor will provide a sign-in sheet of attendees and training material to Fresno Regional Office by 4/23/2026. This incident was also self reported to CCLD. This requirement was not met by: Based on interviews and record reviews, it was revealed that staff left child unsupervised from multiple minutes which is an immediate risk to the personal rights, health, and safety to the children in care. 8

The Director will also make sure there is enough light in the facility during naptime to form to a parent/ authorized representative during naptime which poses a potential risk to the health, safety, and personal rights of children in care. 8 provide staff with the ability to properly identify parents and authorized representatives.
