101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs…(1) No child(ren) shall be left without the supervision of a teacher at any time…Supervision shall include visual observation. This requirement was not met by: 1 Per Center Supervisor, an active supervision training was conducted on 2/13/2026. Center Supervisor also provided a sign-in sheet of attendees and training material. Therefore, deficiency plan of correction was completed prior to today's inspection. LPA received a copy of training adenda and attendance sheet. Based on interviews and record reviews, it was revealed that staff left child unsupervised for approximately which is an immediate risk to the personal rights, health, and safety to the children in care. 8
