Rusd/Harada Elementary School

Based on record review and interview, the licensee did not comply with the section cited above in that 3 out of 4 staff records (Staff 1 [S1], Staff 2 [S2], & Staff 3 [S3]) did not have a Health Screening available to review. Additionally, 1 out of 4 staff (S2) did not have proof of their immunity to Measles available to review. This poses/posed a potential health, safety or personal rights risk to persons in care.

101700.3 (b)(1): A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: 1 Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Facility will notify CDSS with completion within 30 days. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets: Outlet A (68 ppb), Outlet B (5.6 ppb), Outlet C (6.5 ppb) and Outley E30 (8.2 ppb). This is a potential health and safety risk to persons in care. 8 Facility is currently waiting on getting a water filtration system installed and will be retesting the water outlets for future use.

Based on observation, interview; record review, the licensee did not comply with the section cited above in there are 4 kitchen outlets, used to prepare meals for preschool children, that have not been tested for lead exposure, which poses a potential health, safety or personal rights risk to persons in care.
